Creating a One Page Intro Card

I have been offered a different role at my current company. . Due to the way the offer system works the offer will be coming from my managers manager (as my manager is acting & they haven't identified the replacement). Due to this the offer has to be approved by his manager who is on the board of directors. I am sure the process is that he will just click 'approved' and carry on with his day, as I dont have interaction at that level. I have taken him on walkthroughs of projects when the executive committee has visited our sites but with minimal interaction, although he appeared very personable. The main reason though that I accepted this offer is because the org of this team is flatter and my reporting is closer to higher levels even if it is considered a lateral move. I wanted to send him a quick one page 'talent card' to introduce myself so he can put a face to the 'click'. What should i include for most impact. I want it to be bold and concise and show me in a good light. Company size is 90K so who knows if he will even read it.
